vgsp: setup-page demand decoupled from hold (demand rows even at hold=0), immediate stale-ring heal at re-registration, walk-fallback USERD re-resolution (r496b FB sweep retries via svc_walk_vaspaces) + userdp diagnostics - ~12s churn ROOT-CAUSED+FIXED: hold_ms=20000 blocked the ring behind the unreadable setup entry (with 0: 458.9 FPS, submissions=7784 vs 0..112); MME wall unchanged (zero 0x114/0x118/0x11c/0x120 hits anywhere; 8GB+FB-shadow scans zero); GATE 1 RED (nonblack 765/921600, 1-row strip); next: correlate pinned client CPU pages (0x7f757af31000->0x12d800000) with GVA families by content parse, pin GVA->PA - SESSION 153 r1158-r1159
