vgsp-sema r618: a failed/refused GR forward now delivers a synthetic channel error edge (notifier status=0xffff + channel event, sema_refusal_fault=1 default) - proven live by the r616 grfaultpoke test: the r616b matmul wedge ended with CUDA_ERROR unspecified launch failure instead of spinning forever; rollback = sema_refusal_fault=0
